Position Overview

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) who is passionate about making a real difference in the lives of residents every day. In this role, you will provide hands-on nursing care, administer medications and treatments, and play a key role in monitoring resident health and ensuring timely, high-quality care. As a valued member of our care team, you will collaborate closely with nurses, CNAs, and providers to deliver safe, compassionate, and person-centered care.

Key Responsibilities

  • Deliver direct nursing services aligned with individualized care plans, physician directives, and established clinical protocols
  • Conduct ongoing evaluation of resident status, documenting findings and promptly reporting any changes to the appropriate members of the care team
  • Prepare and administer prescribed medications and therapeutic interventions in accordance with regulatory standards and scope of practice
  • Cook up with interdisciplinary team members to support continuity of care and achieve optimal clinical outcomes
  • Provide education and guidance to residents and their families regarding treatment plans, medications, and overall health management
  • Oversee and direct the work of CNAs and support staff, ensuring care is delivered safely, effectively, and in accordance with established standards
  • Complete thorough and accurate documentation within the electronic medical record system in a timely manner
  • Contribute to facility-wide initiatives related to infection prevention, quality improvement, and resident safety
  • Identify and respond appropriately to urgent or emergent situations, following established clinical procedures
  • Maintain compliance with professional standards, confidentiality requirements, and all applicable federal and state regulations

Qualifications

  • Graduate of an accredited practical nursing program with a current, active LPN license in good standing with the State of Wyoming
  • Current Basic Life Support (BLS) or CPR certification
  • Demonstrated ability to deliver compassionate, resident-focused care across a variety of clinical conditions and acuity levels
  • Proven ability to work collaboratively within a multidisciplinary healthcare environment
  • Strong critical thinking and decision-making skills with the ability to adapt to changing clinical situations
  • Effective communication and relationship-building skills with residents, families, and healthcare professionals
  • Commitment to delivering high-quality care and improving the overall well-being of the senior population
