LPN Resident Care Nurse
Position Overview
We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) who is passionate about making a real difference in the lives of residents every day. In this role, you will provide hands-on nursing care, administer medications and treatments, and play a key role in monitoring resident health and ensuring timely, high-quality care. As a valued member of our care team, you will collaborate closely with nurses, CNAs, and providers to deliver safe, compassionate, and person-centered care.
Key Responsibilities
- Deliver direct nursing services aligned with individualized care plans, physician directives, and established clinical protocols
- Conduct ongoing evaluation of resident status, documenting findings and promptly reporting any changes to the appropriate members of the care team
- Prepare and administer prescribed medications and therapeutic interventions in accordance with regulatory standards and scope of practice
- Cook up with interdisciplinary team members to support continuity of care and achieve optimal clinical outcomes
- Provide education and guidance to residents and their families regarding treatment plans, medications, and overall health management
- Oversee and direct the work of CNAs and support staff, ensuring care is delivered safely, effectively, and in accordance with established standards
- Complete thorough and accurate documentation within the electronic medical record system in a timely manner
- Contribute to facility-wide initiatives related to infection prevention, quality improvement, and resident safety
- Identify and respond appropriately to urgent or emergent situations, following established clinical procedures
- Maintain compliance with professional standards, confidentiality requirements, and all applicable federal and state regulations
Qualifications
- Graduate of an accredited practical nursing program with a current, active LPN license in good standing with the State of Wyoming
- Current Basic Life Support (BLS) or CPR certification
- Demonstrated ability to deliver compassionate, resident-focused care across a variety of clinical conditions and acuity levels
- Proven ability to work collaboratively within a multidisciplinary healthcare environment
- Strong critical thinking and decision-making skills with the ability to adapt to changing clinical situations
- Effective communication and relationship-building skills with residents, families, and healthcare professionals
- Commitment to delivering high-quality care and improving the overall well-being of the senior population
