Essential Duties And Responsibilities
- Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
- Establishes open lines of communication with consultants.
- Has effective interviewing skills for the purpose of collecting information from patients, families and staff members.
- Aids Director with patient or family interviews for assessment purposes.
- Aids Director with evaluation of each patient's condition and medical data to determine special needs, or the need to amend the admission assessment.
- Maintains a comprehensive understanding of nursing resource material and understands how to incorporate such information into care plans.
- Establishes open lines of communication with interdisciplinary team members in order to provide a comprehensive and holistic patient care plan.
- Coordinates schedule with nursing center staff and others (family, therapists, etc.) to provide for appropriate participation.
- Aids in ensuring that care is provided in accordance with patient’s wishes.
- Aids in coordinating the development of a care plan (preliminary and comprehensive) for each patient that identifies problems/needs, appropriate interventions and measurable goals.
- Aids in ensuring that care plans are completed in a timeframe that is in accordance with current regulations.
- Aids in providing that care plans are reviewed and administered correctly.
- Aids in monitoring the MDS process for accuracy according to the MDS guidelines and other accepted standards.
- Aids in educating the interdisciplinary team and other staff members as needed in the MDS process.
- Aids in facilitating physical assessments of patients to provide accurate MDS process as requested.
- May be requested to assist with nursing service educational programs in reference to patient assessment and care.
- May be requested to assist with meeting all shifts for the purposes or planning, coordinating and implementing patient care.
- Aids in compliance with Federal, State and local regulations in relation to scheduling, performing and implementing patient care plans.
- May be required to assist with management reports and participate in special assignments.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
Skills And Abilities
- Proficient in using a computer, especially Microsoft Office, Excel and Power Point.
- Excellent organizational and prioritizing skills required.
- Effective and professional verbal, and written communication skills.
- Communicates well with other associates, patients, family members, and visitors providing warm and friendly greeting and an approachable attitude.
- Responds to expressed concerns while displaying a helpful, caring demeanor, answers questions when appropriate in a professional manner.
Minimum Qualifications
- Nursing Degree from an accredited college or university or is a graduate of an approved LPN program.
- An active LPN License to practice in the State of Georgia.
