LPN - PRN

Job Summary

The Terraces of Phoenix, a premier 5-Star HumanGood life plan community, is seeking a compassionate PRN Licensed Practical Nurse (LPN/LVN) for our Skilled Nursing Team. PRN/Per diem: $38.00 - $45.00/Hour (DOE).

Responsibilities

• Ensure a safe and healthy environment for residents, staff, and visitors.
• Participate in the resident’s daily life rhythm.
• Develop assignments in coordination with the care partners.
• Keep each team member accountable for resident care and satisfaction.

Requirements

• Current LPN license in the state of Arizona.
• Current CPR Certification.
• Experience in skilled nursing, subacute care, admissions, or case management is highly desirable.
• Excellent assessment, documentation, communication, and organizational skills.
• Strong customer service and teamwork acumen.
• Current or prior experience with PCC is preferred.
