About the role

At St. Croix Hospice, the Licensed Practical Nurse provides direct patient care under the supervision of a Registered Nurse. This role includes following each patient’s individualized Plan of Care, delivering treatments, and collaborating with the care team to support hospice patients, provide comfort, and enhance overall quality of life. Responsibilities

• Provide direct patient care and follow the Plan of Care to support end-of-life needs.
• Document all care accurately and promptly while educating patients and caregivers.
• Coordinate care by communicating patient needs to the interdisciplinary team.
• Manage pain and symptoms, use equipment safely, and maintain full HIPAA compliance.

Requirements

• Holds current Licensed Practical Nurse (LPN) license in state(s) of practice.
• Graduate of an accredited practical nurse or vocational nursing program.
• Ability to pass background check, consumer report, and drug test.
• Reliable transportation, valid driver’s license, and proof of auto insurance.

Schedule

Full-time position: Monday–Friday, 8:00 AM to 5:00 PM, with weekend visit coverage as needed.
