About the Role
We are seeking a licensed Practical Nurse (LPN) passionate about home care to join a dedicated Skilled Nursing team.
Responsibilities
- Travel to clients' homes to provide personalized care based on physicians' orders
- Perform assessments, monitor client conditions, and document observations, interventions, and evaluations
- Follow up and execute care plans with compassion and professionalism
Requirements
- Valid U.S. nursing license
- Graduation from an accredited nursing program
