About the role
LPN Primary Care professional in a tech-enabled primary care team.
Responsibilities
- Provide direct patient care such as vital signs, injections, wound care, and point-of-care testing.
- Support physicians and NPs during exams and procedures.
- Manage triage through phone and digital communication tools.
- Document thoroughly in the electronic medical record (EMR).
- Educate patients on treatment plans and preventive care.
- Contribute to quality improvement initiatives and optimize workflows.
Qualifications & Skills
- Licensed Practical Nurse (LPN) credential.
- Strong communication and patient education skills.
- Proficiency with EMR systems and digital tools.
- Ability to work collaboratively in a team-oriented, innovative environment.
