About the role

The LPN Clinic is responsible for providing patient care following established standards and practices. Assists physician, nurse, and staff with the daily coordination of patient care and patient access. Coordinates patient flow, schedules appointments, procedures, tests and referrals. May serve as a preceptor for clinical staff. Relies on guidelines to accomplish tasks. Relies on some experience and judgment to complete job. Works under general supervision. May lead and direct the work of others within scope of practice.

Sign-On Bonus: $2,000

Responsibilities

  • Patient Care:
    • Direct patient care is provided utilizing the nursing process under the direction and supervision of the healthcare provider at all times.
    • Diagnostic test results are reviewed and abnormal results and changes in patient condition are reported to the RN, physician and appropriate healthcare team members immediately.
    • Prescribed medications, therapies and treatments are administered in accordance with practical nursing standards at all times.
    • The environment to promote patient and healthcare team safety is evaluated and maintained at all times.
    • Compassionate basic nursing care is provided while maintaining patient privacy and confidentiality at all times.
    • Components of the plan of care within the LPN scope of practice and competency are implemented to promote desired outcomes at all times.
  • Coordination of Care:
    • The execution patient's plan of care is effectively collaborated with the supervising healthcare provider at all times.
    • Physicians, RNs and health care team members are effectively assisted with data collection, assessment, planning, intervention and evaluation of patient and families care to achieve desired outcomes.
    • As an advocate for the patient, supports the patient's psychological, ethical, spiritual and cultural needs by collaborating with the RN and healthcare team.
    • Healthcare team members are sought out to serve as a resource to increase practice knowledge.
    • Active participation on unit teams, work groups and/ or committees is provided as requested; anticipates needs and voluntarily assists others.
  • Quality:
    • Knowledge of unit-based quality improvement initiatives is demonstrated at all times.
    • Active participation in Quality improvement initiatives is provided by utilizing standards, guidelines, and pathways for care delivery.
  • Other Duties as Assigned: Other duties as assigned are completed.

Qualifications

  • Graduate of Accredited LPN program
  • High School
  • Proficient in English, verbal and written communication skills and computer skills
  • Licensed as LPN in state of Louisiana or work permit and BLS certification
