About the Role

We are seeking Licensed Practical Nurses (LPN) to provide one-on-one home health care for medically fragile children and adolescents.

Responsibilities

• Administer medications, treatments, and therapies according to physician plans.
• Conduct ongoing patient assessments and coordinate care.
• Educate families on clinical care to promote positive outcomes.
• Focus on the medical needs of patients primarily in their homes.

Requirements

• Active LPN/LVN license.
• Ability to provide care in home settings.
• Positive, professional attitude.

Schedule
