About the Role

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care in patients' homes, following physicians' prescriptions.

Responsibilities

• Deliver high-quality clinical services within the scope of practice, adhering to state laws and infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, and healthcare team members.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• CPR certification.
• Reliable transportation and a valid driver’s license.
• Ability to work independently in diverse home environments.
• Strong communication and organizational skills.
