Responsibilities

  • Provides direct patient care with the established model of nursing as evidenced by the following:
    • Documents admission information under supervision of the Registered Nurse
    • Collaborates with the Registered Nurse and other members of the Health Care Team
      • Collaborates consistently with Registered Nurse to develop the patient's plan of care and implement interventions on all assigned patients
    • Verifies, prepares and administers medication to all assigned patients to ensure safe medication delivery and positive outcomes
      • Clarifies any questions regarding physician's prescription orders immediately to the Registered Nurse 100% of the time
    • Participates in team building activities that contribute to the patient care, staff education, and self development
      • Participates in interdisciplinary conferences
    • Maintains clinical competency to ensure that practice is evidence-based
      • Attends all mandatory and continuing educational classes

Qualifications

  • Graduated from an accredited LPN program
  • Current license in the State of Connecticut
  • Prior experience in a clinical procedural area is preferred
  • Must complete an approved medication course and forty (40) hours of supervised medication administration including medication verification, transcription of physician's prescriptions, preparation and administration of medications in accordance with department policy within one (1) year of employment with Y-NHH
  • Must demonstrate competency in designated hospital and unit physical demands
    • Requires prolonged standing; walking; use of sight, hearing and touch; and moving/lifting patients of varying weights; CPR
