About the role

The LPN is responsible for delivering nursing service duties in accordance with healthcare provider orders while providing oversight to the resident care and Medication Management Program. This role operates in a community setting, offering a relaxing change from a hectic hospital environment. Here, you’ll have the opportunity to build relationships with residents and provide compassionate care.

Responsibilities

  • Work as part of the team within the Health Services Department.
  • Participate in and complete resident assessments to identify care needs.
  • Develop and update resident service plans with person-centered approaches and interventions.
  • Follow up on residents’ health conditions and changes, communicating with healthcare providers, residents, and responsible parties.
  • Perform nursing services as directed, in accordance with the Nurses Practice Act and state regulations.
  • Develop and maintain effective relationships with residents, families, responsible parties, and healthcare providers.
  • Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care.
  • Assist with coordination of care for new move-ins, residents on extended leave, or those returning from alternate levels of care.
  • Complete documentation via Yardi EHR according to SRG policy and state regulations.
  • Initiate internal incident reports and state reports as needed; participate in incident investigations.
  • Coordinate residents’ incidental medical, dental, vision, hearing, podiatry care, and/or telehealth visits.
  • Coordinate onsite lab diagnostic testing services and/or durable medical equipment.
  • Complete Long Term Care insurance verification forms as needed.
  • Support the Medication Management Program, focusing on med room systems, medication delivery, storage, and security.
  • Collaborate with preferred pharmacy partners.
  • Provide residents with ADL care and assistance with medication administration as needed.
  • Assist the Director of Assisted Living with daily staffing issues as needed.

Requirements

  • Must be a Licensed Practical Nurse (LPN) and/or Registered Nurse (RN) holding a valid nursing license in good standing.
  • Minimum of one (1) year of experience in a senior living environment preferred.
  • First Aid and CPR certification required.
  • Must pass a criminal background clearance before beginning work.
  • Proficient with Microsoft Office, eMAR, and other related software.
