St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.
Responsibilities
- Accurate assessment of each patient’s status, problems and needs.
- Collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an ongoing basis.
- Provides PRN visits on patients who call the answering service.
- Provides crisis care visits as assigned.
- Provides PRN visits as requested by PCC or MD.
- Provides care for patients for the entire County.
- Works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.
Requirements
Current and active Florida (LPN) License required.
Qualifications
Hospice, home health, palliative care, or related experience preferred.

$1,500 Bonus
Schedule
3 shifts: 7 pm - 7 am
