About the role
We are seeking a Licensed Practical Nurse (LPN) to support participant care in a community-based setting.
Responsibilities
- Manage an assigned caseload in collaboration with the interdisciplinary team.
- Monitor participants for changes in condition, hospitalizations, ER visits, falls, wounds, medication concerns, and psychosocial issues, and communicate findings promptly.
- Coordinate nursing visits, support urgent care initiatives, and assist with post-ER and hospitalization follow-up.
- Provide participant and caregiver education on appropriate resource utilization and emergency contacts.
- Participate in home visits, assess participant needs, review medical data, and update care plans accordingly.
- Administer medications, deliver basic care, evaluate responses, and document all activities accurately.
- Support individualized care plans based on assessments, medical orders, and participant input.
Requirements
- Current LPN license in the relevant state.
- At least two years of experience with the geriatric population.
- Proficiency in Microsoft Office and ability to travel locally using a personal vehicle.
- Ability to recognize and address clinical issues proactively.
Schedule
Balanced work environment with no nights, weekends, or holidays.
Pay
Competitive salary range of $74,000 to $88,000 annually.
