Responsibilities
- Perform medication reconciliation of orders with contracted facilities’ Medication Administration Records (MARs) and Electronic Medical Records (EMR) to ensure accuracy and avoid medication errors.
- Coordinate the process for any corrections and ensure follow-up was completed.
- Complete monthly medication reconciliation of facility MARs with InnovAge medication list.
- Communicate medication-related issues to all necessary parties.
- Pull necessary reports throughout the month to ensure accurate orders.
- Document in EMR and other applications as needed.
- Visit facilities monthly to obtain MARs, communicate with staff, and deliver reconciled orders.
- Apply knowledge of medical terminology and pharmaceutical abbreviations.
- Use knowledge of pharmaceutical generic and trade names, dosage forms, and cross-reference common drugs from memory.
- Work with intense and consistent concentration, focusing on detail, speed, and accuracy.
- Work independently and follow through on assignments with minimal supervision.
- Communicate clearly and effectively with pharmacists, nurses, and physicians via telephone and verbally.
Requirements
- Current LPN license to practice in the state.
- 3+ years of clinical experience in a healthcare/long-term care setting.
- Demonstrated clinical assessment skills at the level necessary to meet job requirements.
- Knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines pertaining to PACE.
- Effective interpersonal skills and ability to work with an interdisciplinary care team.
- Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures for quality care.
- Excellent computer skills, including email use.
- Ability to create positive and effective relationships with associates, participants, family members, and external agencies.

Pay
Posted pay range: $31.01 - $38.80 per hour.
