LPN/LVN - PACE

Role Overview The LPN / LVN provides direct nursing care, coordinating with physicians, RNs, and home care teams to ensure participant satisfaction and retention. Responsibilities include taking vital signs, wound care, medication management, treatments, patient education, venipuncture, catheter and foot care, and preparing participants for physician visits. Qualifications & Skills Washington Licensed Practical Nurse licenseCPR and First Aid Certification (American Heart Association)At least 1 year of long-term care or geriatric experienceExperience working with the elderly and frail Key Duties Assist in delivering comprehensive nursing care in a community-based settingCoordinate care with healthcare team membersSupport patient education and treatment plansParticipate in care activities such as wound care and medication administration Additional Information Offers a $5,000 hiring bonus for eligible candidates, competitive pay range ($27.81 - $42.55/hr), and comprehensive benefits supporting professional growth and well-being.
