Responsibilities

  • Provides direct patient care, including monitoring vital signs and assisting with activities of daily living (ADLs).
  • Administers medications as prescribed within scope of practice.
  • Documents patient care, assessments, and interventions accurately and timely.
  • Assists with specimen collection, wound care, and other clinical procedures as directed.
  • Collaborates with the RN in developing, updating, and implementing patient care plans.
  • Monitors and reports changes in patient condition to the RN or physician.
  • Provides patient and family education regarding care plans and discharge instructions.
  • Maintains a safe and clean patient environment in accordance with infection control protocols.
  • Participates in staff training, competency assessments, and quality improvement initiatives.
  • Responds to emergencies and assists in patient stabilization as directed.

Skills

  • Knowledge of basic nursing principles, medication administration, and patient care procedures.
  • Strong communication and interpersonal skills for interacting with patients and the healthcare team.
  • Ability to work effectively in a fast-paced, high-acuity environment.
  • Proficiency in electronic medical records (EMR) systems.
  • Demonstrates professionalism, accountability, and a commitment to patient-centered care.
  • Ability to lift and move patients and equipment as necessary.
  • Strong organizational skills and attention to detail.

Qualifications

  • Graduate of an accredited Practical Nursing Program required.
  • Current state LPN/LVN licensure required.
  • Basic Life Support (BLS) Certification as required per facility policy.
  • De-escalation Training as required per facility policy.
  • Minimum of six (6) months of experience in a medical/surgical setting preferred.
