Responsibilities

• Work with Medicare patients throughout their healthcare journey.
• Guide patients through complex health concerns and help achieve their health and wellness goals.
• Identify and prioritize patients' needs to build comprehensive care plans that address social determinants of health.
• Set your own hours and work remotely, interacting with patients via video or phone calls.

Requirements

• LPN/LVN license in good standing.
• Experience working with diverse patient populations.
• Empathy and clinical skills to build strong, trusting relationships and provide holistic care.
• Ability to adapt quickly to new systems and take initiative on tasks.

• Based in the United States.
