About the Role
Provide compassionate, personalized patient care in a rehabilitation setting.
Responsibilities
- Deliver direct patient care according to personalized care plans and physician orders.
- Monitor patient vital signs, symptoms, and responses, reporting findings to RNs.
- Build meaningful relationships with patients to support their physical, mental, and emotional recovery.
- Support patient progress and celebrate victories along their rehabilitation journey.
Requirements
- Current LPN/LVN licensure as per state requirements.
- Preferred experience in inpatient medical-surgical or hospital settings; rehabilitation experience is a plus.
- Strong communication skills and compassionate approach.
