About the role

CareBridge Health is a home and community-based services provider within the Elevance Health family. This role allows for virtual full-time work, with occasional in-person training sessions.

Virtual

This role is virtual full-time, except for required in-person training sessions. It promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

Work Shift

The position is full-time (40 hours a week), requiring holidays and weekend rotation. During the first 2 weeks of training, the schedule is Monday to Friday, 8a-5p central time. Afterward, the regular schedule is working 4 10s, 8a-7p central time. Every other weekend and every other holiday.

Responsibilities

• Reviews patient medical records and interviews patients.
• Records vital signs and provides nursing interventions and coaching in accordance with the scope of practice and standing orders.
• Facilitates patient transfers to an alternate level of care as needed.
• Documents using standard templates.
• Engages with patients via the phone up to 80% of the shift.
• Works assigned weekends per standard team rotation.
• Requires reliable internet and high-level customer service skills.

Requirements

• LPN or LVN license in the applicable state required.
• Minimum of 2 years of experience as an LPN/LVN; or any combination of education and experience, which would provide an equivalent background.
• Satisfactory completion of a Tuberculosis test is a requirement for this position.

Preferred Experience, Skills, And Capabilities

• Bi-lingual (English/Spanish) preferred.
• Experience with Telephonic Triage preferred.
• Acute care experience (e.g., urgent care or ER) preferred.

Pay

The salary range for this specific position is $22.30 p/h to $32.06 p/h. The actual compensation may vary based on geographic location, work experience, education, and/or skill level.

Schedule

Associates are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process.
