About the role

The LPN performs nursing functions within the household, including passing medications, observing residents’ condition, and communicating them to the registered nurse or physician as needed. The LPN works closely with the RN to ensure regulatory compliance, nurture a person-centered and resident-directed culture, strengthen the household’s ability to function as an effective home, and promote individual and team learning. LPNs serve as teachers, role models, and resident advocates for quality clinical care, providing routine daily nursing care aligned with each resident’s assessment, choices, and plan of care.

Responsibilities

• Provide leadership and vision to the household by serving as a role model for core competencies, maintaining professionalism, and supporting a learning environment through continuous education and collaboration.
• Ensure clinical operations comply with federal, state, and local regulations, the Minnesota Nurse Practice Act, and professional nursing standards while honoring person-centered and resident-directed care.
• Monitor residents’ status to assure quality care and coordinate with other resident services for continuity of care.
• Implement clinical policies and procedures that ensure safe, sanitary, and efficient practices, including documentation, medication administration, resident nursing care, safety, sanitation, staff development, charting for RAI process, materials management, CQI, and resident rights.
• Participate in orientation, training, clinical competency assessments, coaching, educating, counseling, and performance appraisals in a manner supportive of team and professional standards.
• Foster collaboration, consensus, and negotiation to address problem-solving in the household; maintain and communicate clinical policies and procedures to all household members, emphasizing resident and family relationships.
• Maintain working relationships with physicians and other medical personnel to support quality clinical care.
• Demonstrate support for organizational fiscal viability through efficient and effective use of household resources, including supplies.
• Maintain a welcoming physical environment and enhance household relationships through shared activities, offering emotional, psychological, and spiritual support to residents.
• Promote residents’ well-being and satisfaction by supporting resident-centered care and participating in interdisciplinary team efforts to establish and implement care plans.

Requirements

• Current MN LPN licensure.
• Three or more years of experience in skilled nursing preferred.
• Training and experience in geriatric nursing preferred.
• Strong interpersonal relationship skills.
• Commitment to providing person-centered care.
• Leadership and supervisory skills preferred.
• Knowledge of federal and state regulations governing clinical care, nursing practice, and leadership.
• Good verbal and written communication skills.
• Ability to work well in a team environment.

Schedule

Monday–Thursday, 10:30 PM–7:00 AM (32 hours per week, 0.8 FTE). No weekends; every other holiday.

Pay

$25.42–$35.59 per hour.
