Licensed Practical Nurse - Long Term Care Job Summary Delivers nursing care to residents in a long-term care setting under the direction of a Registered Nurse or physician. Administers medications and treatments, monitors resident status and supervises Certified Nursing Assistants. Maintains accurate clinical documentation and communicates changes in condition to the care team. Education & Experience Graduate of an accredited practical or vocational nursing program: Required Long-term care or skilled nursing experience: Preferred Licenses & Certifications Licensed Practical Nurse or Licensed Vocational Nurse licensure in the state of practice: Required Basic Life Support (BLS) certification: Required Key Responsibilities Administer medications and treatments per physician orders and facility policy. Perform resident assessments and monitor for changes in physical, cognitive or behavioral status. Supervise and delegate appropriately to Certified Nursing Assistants. Complete accurate and timely documentation in the resident record. Communicate changes in condition to the Registered Nurse, physician and family as appropriate. Follow infection control, safety and regulatory requirements for long-term care.
