About the role

The Licensed Practical Nurse (LPN) is a key member of the multidisciplinary treatment team, delivering compassionate, high-quality nursing care to individuals recovering from substance use and co-occurring mental health disorders. Working under the supervision of an RN or clinical leader, the LPN supports medical coordination, monitors patient health, administers medications, and helps maintain a safe, structured, and therapeutic environment. This role is essential to ensuring patient stability, clinical quality, and a consistent, recovery-focused milieu.

Responsibilities

  • Provide direct nursing care under the direction of an RN or clinical supervisor.
  • Conduct basic nursing assessments and report significant findings promptly.
  • Administer medications and treatments in accordance with physician orders and facility protocols.
  • Monitor vital signs, withdrawal symptoms, and overall patient stability; escalate concerns appropriately.
  • Provide patient education and support as part of discharge planning.
  • Collaborate with the treatment team to support individualized care plans.
  • Participate in interdisciplinary meetings and contribute observations related to patient progress.
  • Serve as a supportive point of contact for patients and families within scope of practice.
  • Assist in monitoring and maintaining medication inventory.
  • Ensure safe storage, accurate documentation, and compliance with medication handling protocols.
  • Support MAR processes, including documentation and reconciliation.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Uphold infection control practices and safety standards.
  • Support program compliance, audits, and survey readiness efforts.
  • Maintain a safe, structured, and therapeutic environment within residential or detox settings.
  • Support crisis intervention and de-escalation alongside clinical and nursing leadership.
  • Assist with additional duties or projects that support overall program operations.

Requirements

  • Active, unencumbered Licensed Practical Nurse (LPN) license in the state of employment.
  • Graduation from an accredited Practical Nursing program.
  • Current CPR/First Aid certification or ability to obtain within 30 days of hire.
  • 1–2 years of experience in behavioral health, substance use disorder treatment, psychiatric care, or residential/inpatient settings.
  • Experience working in a recovery-oriented, trauma-informed environment.

Work Conditions / Physical & Mental Requirements

  • Primarily indoors in a climate-controlled clinical environment.
  • Regular standing, walking, sitting, bending, and repetitive tasks such as documentation or vitals checks.
