About the role

As an LPN Licensed Practical Nurse at Spring Creek Healthcare Center, you will provide high-quality, patient-centered care to our residents. Your weekend shifts are crucial for ensuring seamless continuity of care and meeting the unique needs of our residents.

Responsibilities

• Providing direct patient care, including medication administration, wound care, and vital sign monitoring
• Developing and implementing individualized care plans in collaboration with the interdisciplinary care team
• Aiding with activities of daily living, such as bathing, dressing, and grooming
• Maintaining accurate and detailed records of patient care and services
• Collaborating with other healthcare professionals to ensure effective communication and coordination of care

Requirements

• Current LPN licensure in the State of New Jersey
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
