About the Role
Provide patient-centered, one-on-one care in a home setting, following the client's plan of care while maintaining dignity and independence.
Responsibilities
- Administer medications, feedings, oxygen, and ostomy care, and assess treatment results.
- Document nursing actions, communicate with families, case managers, and physicians, and update on patient progress.
- Coordinate or order supplies, medications, and feedings to ensure availability.
- Ensure all patient needs are met per physician instructions.
Requirements
- Licensed Practical Nurse (LPN/LPT/LVN) degree with current license in practice state.
- Valid driver's license, acceptable driving record, and proof of insurance.
- CPR certification.
- Preferred: 1+ years of nursing or home health care experience.
