LPN / Licensed Practical Nurse - Private Duty (Days)

Schedule

Full‑Time, Days – Sunday through Thursday, 9:00 am – 5:00 pm.

Responsibilities

• Provide 1:1 care for the client following the client’s plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed per the client’s plan of care.
• Document nursing actions, communication with family, case manager, and physicians, and track progress toward established goals.
• Inform physicians, case managers, and families of changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always on hand.
• Ensure all patient needs are met in accordance with physician instructions.

Qualifications

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.
• Adult case trach/G‑tube experience preferred.
