About the role
Ennoble Care is looking for a full-time Hospice Licensed Practical Nurse to work within and around the South Philadelphia, PA area. The role involves providing care and support to terminally ill patients, assisting the Registered Nurse, performing specialized procedures, preparing and implementing the patient plan of care, collaborating with the interdisciplinary group, and reinforcing teaching to the patient/family members.
Responsibilities
- Assist the Registered Nurse in carrying out the plan of care.
- Auxiliary in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.
- Implement, continue to evaluate, and update the patient plan of care.
- Collaborate with the Interdisciplinary Group (IDG) to keep the patient's plan of care current and address any identified issues.
- Help reinforce teaching to the patient/family members regarding end of life care.
- Foster cooperative efforts among personnel by understanding and participating in team and staff conferences.
Requirements
- Graduated from an accredited School of Practical Nursing.
- Licensed in VA as a Licensed Practical/ Vocational Nurse by Virginia State Board of Nursing.
- One or more years of nursing experience, with hospice experience preferred.
- Knowledgeable in caring for the terminally ill, including administering medication and providing emotional support.
- Knowledgeable in safety precautions and high ethical standards.
- Committed to maintaining sound judgment and attention to detail.
- Excellent communication skills, both verbal and in writing.
- Proven experience as a hospice LPN.
- Committed to safety precautions and high ethical standards.
- Solid communication skills and bilingual skills are a plus.
- Compassionate and patient with an emotional stability.
Qualifications
- Must be a licensed driver with an insured automobile in good working order.
