Responsibilities

• Provide skilled, one-on-one nursing care in the home
• Administer and manage medications
• Monitor and document patient conditions
• Perform treatments as ordered by the physician
• Educate and support family members
• Collaborate closely with the Phoenix clinical team

Qualifications

• Active Illinois RN or LPN
• Strong assessment and communication skills
• Compassionate, dependable, and self-motivated
• Valid driver's license and reliable transportation

Pay

• Competitive pay based on experience
• Weekly pay
• Weekend Day Shift
• Premium Pay: Weekday Overnights +$3/hour, Weekend Days +$4/hour, Weekend Overnights +$5/hour
