About the role
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Rochester Residence and Care Center in Rochester, PA.
Responsibilities
- Providing direct patient care under the guidance of a professional registered nurse (RN)
- Administering medications, treatments, and performing various medical procedures as needed
- Monitoring patient vital signs and reporting changes to RNs or physicians
- Assisting with ADLs (activities of daily living) and mobilitizing patients
- Maintaining accurate and up-to-date patient records and reports
- Collaborating with the healthcare team to develop and implement individualized care plans
- Providing emotional support and comfort to patients and their families

Requirements
- Current LPN licensure in the state of Pennsylvania
