LPN Licensed Practical Nurse in Cherry Hill

About the role

We are seeking a dedicated and compassionate LPN to join our team at our Burlington, New Jersey location. As an LPN at Silver Healthcare, you will provide high-quality, individualized care to our patients, work closely with our interdisciplinary team to develop and implement personalized care plans, assist with daily living activities, maintain accurate and detailed records of patient care and treatment, participate in shift reports, and communicate effectively with other members of the healthcare team. You will also provide emotional support and empathy to patients and their families.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and reporting changes in patient condition
• Develop and implement personalized care plans in collaboration with our interdisciplinary team
• Absorb and perform daily living activities, such as bathing, dressing, and grooming
• Maintain accurate and detailed records of patient care and treatment
• Participate in shift reports and communicate effectively with other members of the healthcare team
• Provide emotional support and empathy to patients and their families

Requirements

• Current LPN licensure in the state of New Jersey
• 0-1 year of experience in a long-term care setting or related field
