LPN licensed practical nurse in assisted living part time 3-11pm

About the role

Encore Village of Schaumburg is seeking a Licensed Practical Nurse (LPN) who has a love for working with seniors and a passion for healthcare. New graduate Licensed Practical Nurses (LPN) are welcome. Shift: 3–11 pm.

Responsibilities

• Performs resident assessment through use of direct physical assessment, behavior monitoring, various assessment forms, review of hospital records, laboratory results, resident medical records, and information provided by the interdisciplinary team, resident and/or the resident’s significant others.
• Monitors residents for changes in condition that may indicate progress or deterioration in the resident’s overall status. Notifies physicians and families of changes and transcribes orders received.
• Administers medications according to facility policy and monitors residents for therapeutic and adverse effects. Notifies physician and pharmacy of suspected and actual adverse reactions.

Requirements

• Current LPN License within the State of Illinois
• Graduation from an accredited LPN program
• Current CPR Card
