LPN / Licensed Practical Nurse - Home Health - $5k Sign On

As a Home Health Licensed Practical Nurse, you will provide 1:1 care to make a lasting impact on patients and families. Responsibilities
- Provide 1:1 care for clients following their plan of care while maintaining their dignity and independence.
- Administer medications, feedings, oxygen, and ostomy care as needed per the client's plan of care, and assess results.
- Accurately document nursing actions, care given, communication with family, case managers, and physicians, as well as progress and outcomes for established goals.
- Inform physicians, case managers, and families of changes in the client's medical condition and needs.
- Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always on hand.
- Ensure all patient needs are met in accordance with the instructions of the physician.
Requirements
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and residence.
- Valid driver’s license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- 1+ years of nursing experience (preferred).
- Home health care experience (preferred).
