LPN, Licensed Practical Nurse - Facility Based Nursing

Cooperative Home Care is hiring LPNs to join our nursing team! Are you a compassionate and dependable nurse looking for flexible work in a nursing facility setting? Cooperative Home Care is looking for dedicated LPNs to provide quality patient care. You will work alongside other healthcare professionals in a supportive nursing facility environment while providing hands-on care to residents. SCHEDULE: Weekends; 7:00am - 3:00pm What You'll Do Provide compassionate, hands-on nursing care to facility residents Administer medications and treatments as ordered Monitor residents' conditions and report changes appropriately Complete accurate and timely documentation Assist with activities of daily living and other patient-care needs Communicate effectively with residents, families, and facility staff Follow all nursing standards, policies, and procedures What We're Looking For Current Illinois LPN license Reliable transportation to the facility Strong communication and clinical skills Dependable, compassionate, and patient-focused Ability to work independently while also being part of a team Previous nursing facility or long-term care experience is a plus Why Join?
