About the Role
As an LPN at Vantage, you will be responsible for:
- Providing direct patient care to our residents, including administering medications, wound care, and treatments
- Monitoring and reporting changes in residents' conditions to nursing staff and physicians
- Collaborating with the healthcare team to develop and implement individualized care plans
- Maintaining accurate and detailed records of residents' care and progress
- Participating in the implementation of individualized care plans and providing support to residents and their families
