We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated professionals. As an LPN at Highfield Gardens, you will play a vital role in delivering exceptional care to our residents and supporting them on their journey to recovery.

Responsibilities

• Provide direct patient care to our residents, including administering medications, wound care, and other treatments as prescribed by physicians
• Monitor and report changes in patient condition to the nursing team and other healthcare professionals
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date records of patient care and treatment

Requirements

• Current LPN licensure in the state of New York
• Prior experience as an LPN preferred; graduates are welcome
• Strong clinical skills and ability to work effectively in a fast-paced environment
• Excellent communication and interpersonal skills
• Ability to work collaboratively as part of a multidisciplinary team
