About the role
We provide skilled nursing care to people recently discharged from the hospital, needing therapy, and those needing long-term care.
Responsibilities
- Administer Medications: Give oral, topical, IV, injectable medications
- Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings
- Monitoring: Observe physical and mental health status - track changes & analyze data
- Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.
- Care Planning: Assist in executing individualized care
- Collaboration: Strategize care with physicians, therapists, social workers & others
- Caregiver Education: Explain health status, changes and expectations for care
- Documentation: Keep a detailed record of health status, all care, progress & interactions
- Team Oversight: Oversee & delegate CNA’s ensuring quality care/teamwork
Requirements
- Licensed to work within the State of Delaware as a LPN
- Able to pass background check, drug screen, professional references check & physical evaluation

- Professional & Clinically Skilled with leadership qualities
Skills
- Proficient in administering medications
- Skilled in wound and skin care
- Experience in monitoring and documentation
- Ability to collaborate with healthcare professionals

Pay
- $3,750 Sign on Bonus for Full-Time
- $2/hr bonus for No Benefits
