Responsibilities

• Administer Medications: Give oral, topical, IV, and injectable medications
• Wound/Skin Care: Perform wound care and prevention, manage pain, apply dressings
• Monitoring: Observe physical and mental health status, track changes, and analyze data
• Clinical Procedure: Provide catheter and ostomy care, gather specimens
• Care Planning: Assist in executing individualized care plans
• Collaboration: Strategize care with physicians, therapists, social workers, and others
• Family & Resident Education: Explain health status, changes, and expectations for care
• Documentation: Keep detailed records of health status, care, progress, and interactions
• Team Oversight: Oversee and delegate to CNAs, ensuring quality care and teamwork

Requirements

• Licensed to work within the State of Connecticut as a Licensed Practical Nurse (LPN)
• Able to pass background check, drug screen, professional references check, and physical evaluation
• Professional and clinically skilled with leadership qualities
