LPN Licensed Practical Nurse

About the role

Nestled in the tranquil, picturesque setting of Avon, CT, Apple Rehab Avon stands as a family-oriented, skilled nursing, and short-term rehab facility offering personalized care in our 60-bed establishment. Situated conveniently just a few miles from Rt. 44, our location ensures accessibility within this idyllic rural landscape.

Responsibilities

• Efficiently plans and provides professional care while aligning with the needs of residents and the changing medical landscape.
• Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.
• Documents crucial information related to resident care accurately and comprehensively.
• Administers prescribed medications and oversees accurate record-keeping in line with facility policies.

Requirements

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).
• In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.
• Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively.
• Collaborative team player with previous experience in long-term care preferred.

Additional Expectations

• All LPN positions require weekend on-call availability to support both staff and residents.

Pay

Starting Pay: Competitive rates up to $37 per hour, based on experience
Differentials Offered: Shift Differentials for 2nd and 3rd shifts and weekends
Additional Shifts Differential: Extra $5.00/hour for any shifts worked outside scheduled hours

Schedule

Available Shifts: 24-hour 3-11 PT position
