Job Summary

Are you a motivated LPN with home health experience who thrives in a fast-paced environment? Our growing home health agency is seeking a dynamic LPN Intake Coordinator to join our dedicated team in Maitland, FL.

Responsibilities

• Answering phones and managing fax communications
• Entering patient referrals into the EMR system
• Aiding with coordination of home health services
• Creating, modifying, and maintaining forms and records
• Filing, distributing Medicare paperwork, and tracking physician orders
• Assembling admission packets
• Supporting quality assurance and regulatory compliance efforts
• Jumping in to help your team whenever needed

Qualifications

• Active LPN license (required)
• Valid driver’s license & reliable transportation (required)
• 1+ year of home health experience (required)
• Experience scheduling home health referrals (preferred)
• Proficiency with Microsoft Office Suite (Word, Excel, etc.) (required)
• Ability to participate in on-call rotation (required)
