Responsibilities
- Provide hospice care services to patients and their families
- Manage patient needs including medication administration, symptom management, and emotional support
- Coordinate services with other healthcare professionals and community resources
- Document patient care and communicate effectively with family members and other caregivers
Requirements
- Bachelor's degree in a related field (e.g., nursing, social work)
- Current certification in hospice and palliative care (if applicable)
- Experience in healthcare or related field preferred
Qualifications
- Strong communication and interpersonal skills
- Ability to handle sensitive and confidential information
- Flexibility to work evenings, weekends, and holidays as needed
