LPN II (VBH Centra Specialty, LTAC, PT, 7a-7p)
About the Role
Provide patient care under the direction or supervision of a licensed medical practitioner, a professional registered nurse, or other licensed health professional. The LPN coordinates and manages the care provided to assigned patients of all ages based on the unit care delivery model, requiring professional knowledge and consideration of specific patient conditions and treatment.
This 26-bed Long Term Care Hospital specializes in caring for medically complex patients who require extended hospitalization and advanced clinical care. Typical cases include individuals recovering from critical illness who need ongoing respiratory support, complex wound care, intravenous therapies, cardiac monitoring, dialysis, and comprehensive rehabilitation services before transitioning to a lower level of care.
We utilize the LEAF system, a wearable patient monitoring technology that helps staff track and document patient repositioning. Our unit provides dialysis treatments at the bedside using TABLO dialysis machines, allowing patients to receive dialysis without leaving their room, improving comfort and continuity of care.
Centra Health is a regional, nonprofit healthcare system located in Central Virginia, comprised of hospitals, medical centers, and a network of specialty and primary care practices.
