About the role
Provide hands-on nursing support to promote the health and wellness of individuals in licensed facilities. Attend Community Participation Supports programs, ensuring personalized and compassionate care aligned with the mission to Care, Comfort, and Heal for adults with intellectual disabilities.
Responsibilities
- Deliver nursing care to residents, including those with tracheostomy and Type 1 Diabetes.
- Coordinate with internal and external stakeholders to facilitate communication and care plans.
- Utilize Electronic Medical Record systems for documentation and administration.
- Support individuals with intellectual disabilities through healthcare and supportive services.
Qualifications & Skills
- Licensed Practical Nurse (LPN) with 3+ years' experience in relevant clinical settings.
- Experience working with individuals with intellectual disabilities.
- Proficiency in basic computer skills and electronic medical records.
- Excellent communication and interpersonal skills.
- Valid PA driver's license and satisfactory background checks, including criminal and FBI clearances.
