About the role
We are seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to pediatric and adult patients in their homes.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process
- Provide care according to prescribed orders
- Monitor signs and symptoms to identify physiologic and psychosocial changes
- Collect and analyze patient data and document nursing diagnoses
Requirements
- Licensed as an LPN/LVN in the practicing state
- Current TB or Chest X-Ray
- Current BLS card
Schedule
- Day or night shifts available
- Shift lengths: 8, 10, or 12 hours
Skills
- Strong assessment and communication abilities
- Ability to adapt care to diverse patient acuities, including G-tube, Trach, and Vent cases
