About the role
We are seeking a Licensed Practical Nurse (LPN) to provide home healthcare services in the Alexandria, OH area.
Responsibilities
- Plan, implement, and evaluate patient care using the nursing process
- Initiate and revise care plans based on patient needs
- Administer medications and treatments, monitoring outcomes
- Respond to patient condition changes and report to physicians or supervisors
- Provide healthcare education, care coordination, and discharge planning
Qualifications
- Current LPN license in the practicing state
- Valid health certificate, PPD or Chest X-ray, and BLS certification
- Preferred: one year of LPN experience
