About the role
We are seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and adult patients. In this role, you will utilize the nursing process to plan, implement, and evaluate patient care.
Responsibilities
- Initiate and adjust care plans based on patient needs
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to physicians or supervisors
- Identify opportunities for healthcare education, care coordination, and discharge planning
Requirements
- Current LPN license for their state
- Valid health certificate
- Current BLS certification
- PPD or Chest X-ray
Qualifications
- At least one year of LPN experience
