About the role
Licensed Practical Nurse (LPN) providing in-home care for pediatric and adult patients.
Responsibilities
- Assessing, planning, implementing, and evaluating patient care using the nursing process.
- Delivering skilled nursing services per prescribed orders.
- Monitoring signs and symptoms to identify physiological and psychosocial changes.
- Collecting, analyzing, and documenting data to determine nursing diagnoses.
Qualifications
- Current LPN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
