About the Role

Provide in-home skilled nursing care for pediatric and adult patients, utilizing the nursing process to assess, plan, implement, and evaluate patient care.

Responsibilities

• Deliver skilled nursing services per prescribed orders
• Monitor physiologic and psychosocial changes in patients
• Collect, analyze, and document healthcare data and diagnoses
• Collaborate with the healthcare team to evaluate patient conditions

Requirements

• Current LPN/LVN license in the practicing state
• Valid TB test or Chest X-Ray
• BLS certification
• Preferred minimum one year of professional nursing experience
