About the role
Provide patient care in a home setting under the supervision of an RN, utilizing the nursing process to assess, plan, implement, and evaluate care.
Responsibilities
- Conduct focused interviews with patients
- Identify patient needs
- Monitor physiological and psychosocial changes
- Document nursing diagnoses
- Additional duties may be assigned as needed
Requirements
- Current LPN license
- Health certificate
- PPD or chest X-ray
- BLS certification
- Prior LPN experience preferred
