LPN – Home Health & Hospice

Your Schedule

• Full-time position
• Two weekends per month
• A weekday off following each weekend you work
• Rotating on-call coverage shared across the team

What You'll Do

• Deliver skilled nursing care across both Home Health and Hospice patients, gaining exposure to two distinct sides of nursing
• Follow physician-directed plans of care and document visits accurately
• Administer medications and treatments within your scope of practice
• Educate patients and families on care plans, medications, and what to expect
• Communicate changes in patient condition to the care team promptly
• Participate in the team’s on-call rotation, providing after-hours support as needed

What We're Looking For

• Active LPN license in Oklahoma
• Experience in home health, hospice, or a related clinical setting preferred
• Current CPR certification
• Comfortable working independently and managing your own schedule
• Adaptable nurses who enjoy variety and want to build skills across both specialties

Oklahoma City and surrounding communities including Edmond, Norman, Moore, Yukon, Midwest City, Mustang, Guthrie, Choctaw, Wellston, Harrah, Piedmont, and Blanchard.
