LPN, Home Health (FT, Days, Lackawanna and surrounding counties)
GENERAL SUMMARY
The Licensed Practical Nurse provides direct and indirect nursing care under the supervision of a Patient Care Supervisor. Responsibilities include administering care to patients and performing various patient care duties. The role adheres to professional working policies and addresses the needs of patients across different age groups.
EDUCATION
Graduate of an accredited Licensed Practical Nurse Program required. A pharmacology course of at least forty-five hours must be included in the basic education curriculum or attended post graduation.
WORK EXPERIENCE
Home Health Experience Preferred. Med-surg or rehabilitation experience preferred. Minimum 2 to 3 years experience as a LPN preferred.
SPECIAL EMPLOYMENT REQUIREMENTS
- Current licensure as a Practical Nurse in the Commonwealth of Pennsylvania.
- Maintain standards of nursing care and implement Home Health division and Allied Services policies.
- Criminal record check clearance by PA State Police.
- Good verbal and written communication skills and interpersonal skills required.
- Valid PA driver’s license and availability during all working hours.
- Maintain confidentiality of patient information.
PHYSICAL DEMANDS
- Moderate Work: duties requiring up to 25 lbs. of force occasionally and up to 10 lbs. of force frequently.
- Requires bending, reaching, stooping, and lifting (occasionally in strained positions).
- Able to transport self throughout facility.
- May need to travel to patient’s homes as required.
ACCIDENT OR HEALTH HAZARDS
- Occasional exposure to blood and body substances.
- Potential exposure to needle sticks.
WORKING CONDITIONS
Works in a well-lit office environment and in varying conditions in the patient’s home (conditions may be less than ideal).
