About the role
The Licensed Practical Nurse in Home Health provides high quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team, following all applicable infection control procedures.
Responsibilities
- Provides thorough documentation of patient visits in the electronic medical record utilizing the agency's electronic visit verification program (as applicable)
- Participates actively in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinators, community case managers (as applicable)
- Functions independently in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Requirements
Current and unrestricted LPN licensure in state of practice
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
Preferred qualifications include current CPR certification or ability to complete within 90 days of hire, home care experience, ability to manage multiple tasks simultaneously, good communication, writing, and organizational skills, and ability to work independently and flexibly.
Qualifications

Pay
Pay Range: $54,323 - $97,100 annual total cash target pay
Per visit point rate compensation
Hourly rate for certain non-visit activities such as orientation
Schedule
Flexible hours as required to meet identified client needs
